The use of microscopes in surgery, i.e. the concept of microsurgery has been pretty common in otology for years. However, no significant improvement has taken place in the processes for years. Endoscopes have been practised for sinus surgeries in otorhinolaryngology, i.e. diseases that concern the nose, ears and throat. Their introduction in otology has been gradual. The endoscopes had been initially used to get a clear image of the middle ear. However, their usefulness soon led to their usage in all surgical procedures that involved the middle ear.
Endoscopic surgeries concerning the ear became popular in the 1990s and are even used as standalone instruments. Furthermore, the use of scarless endoscopic tympanoplasty would mean that there would be no formation of scar tissues.
Why is scarless endoscopic tympanoplasty used?
Tympanoplasty is a common procedure that is used for treating chronic otitis media. The tympanoplasty includes complete removal of any disease in the middle ear. The perforated tympanic membranes are repaired and hearing of the patient is restored. The common techniques that have been used in terms of tympanoplasty include postauricular, endaural and transcanal. The transcanal procedures are more common in recent days. Endoscopic tympanoplasty is preferred because it offers a large field of view.
The wide field of view means a better visual. The ossicles and the middle ear can be seen quite well through the perforation. The convoluted anatomy of the outer ear and the bone protrusions affect the microscopic processes adversely. The visualization of the deep structures is impaired. This makes the panoramic and wide-angle views of the endoscopic methods very preferable. The back and forth movements of the endoscope is not affected by the convoluted anatomy of the external ear canal.
